Our catalog of forms and supplies include items for every department in your dealership. Most of the forms are on the shelf ready to ship. ORDERING METHODS

Orders may be submitted via fax at (251) 634-9423, or toll free at (888) 343-6006, or via email at charles@formsolutions.com, or through our on-line shopping cart. Request your free Dealership Products Catalog.

INSTRUCTIONS

Select product, select quantity, and add to basket. Use save order option to easily save product(s) for future orders. Go to store front to continue shopping. Proceed to checkout when finished. Create New Account saves your account information for future orders, or you have the option to place order without saving account info. Under Payment Selection enter “Check” to be invoiced or “MasterCard” or “Visa” to pay with credit card. Credit card info is not retained & must be entered with each order. Shipping charges are added after your order ships.

SHIP TIME

Non-imprinted forms usually ship within 24 hours. Imprinted forms ship in 2 to 3 weeks.

IMPRINTED FORMS

Most forms can be imprinted with numbering & up to 4 lines with your dealer name, address and phone number. Minimum quantity for imprinting is 500 and cost is $50.00. Some forms include imprinting. Your imprint can be input at checkout. Logos are an additional $50.00 & must be emailed to charles@formsolutions.com. We are able to accept your camera ready artwork in many formats but EPS is preferred.

PRIVACY NOTICE

Form Solutions will not sell or give your account, email & credit card info to anyone other than what is needed to ship & bill your order.

RETURN POLICY

Non-imprinted forms can be returned with a return authorization number. There is a 20% restocking charge & no refund on shipping charges. Imprinted forms cannot be returned. If your order is incorrect, it will be replaced.
